Bain's Circuit System-7 :
A Revolutionary Concept for Spontaneous and controlled
Respiration for Adult as well as Paediatric Anaesthesia
INTRODUCTION
Anaesthetic Circuits are primarily designed to overcome the problem of rebreathing which lead to CO2 accumulation. The Bain's circuit is the only ideal circuit for spontaneous, controlled or assisted respiration, used for adult as well as for paediatric anaesthesia.
Modified system
Medisys Bain's Circuit System-7 is the modified combination of Mapleson 'D' and 'E' Circuits. Constituent of Circuit
The circuit consists of an Expiratory and a separate Inspiratory limb placed co-axially.
Dimension of Limb
The Expiratory limb consists of a 1.8 meter long corrugated tube with an uniform diameter of 22mm and an internal volume of 500ml.


Mechanism
The inspiratory limb being smaller in diameter, is incorporated in the expiratory limb for delivering gas at the patient's end.
• The Inspiratory tube is fixed to an inlet Nipple placed at an angle near the Expiratory end.
• Being centrally fixed inside the corrugated tube in coaxial manner, it ends just short of the patient's end.
• The Inspiratory/ Patient's end of the circuit connects firmly to a Face Mask of directly to an endotracheal Tube.
• The Expiratory End remains free for spontaneous breathing.



INFLOW OF GAS
Fresh inflow of gas from the Anaesthetic Machine to the Inlet Nipple of the circuit is brought by a rubber tube.

NEED FOR PRESSURE

When a pressure build up is desired in the Expiratory limb during spontaneous, assisted or controlled ventilation, a Bag-Mount with a Heidbrink valve is attached to the Expiratory End of the Circuit.
WORKING SYSTEM

Gas flows along a straight tube. The pattern of flow conforms to series of Concentric Cylinders of gas arranged in a telescopic fashion as shown in the figure.
• Therefore, the peripheral cylinder remains stationary and the central Cylinder moves very fast.
• If the composition of the Gas entering a laminar Flow in suddenly changed, the new gas advances down the tube in a Cone Front.
• The advancing Cone Front of the Laminar gas flow means that some gas reaches the end of the tube before the total amount of gas.
MODIFIED CIRCUIT BENEFIT
• In this modified circuit, because of the co-axial placement of the Inspiratory Tube inside the expiratory limb, the fresh gas flow is delivered directly to the patient in a Cone-fashion without any resistance or turbulence.
• The tip of the cone continuously flushes the alveoli even at much lower flow rate.
• During Expiratory pause this continuous jet stream of fresh gas rapidly fills up the patient's end of the expiratory limb.


• During subsequent inspiration, the patient is delivered the fresh gas flowing from the inner tube along with a mixture of fresh gas and exhaled Co2 which is lying in he Outer expiratory limb.

OBSERVATION
It has been observed that inflow of 70ml/kg Body wt/Minute (50ml/kg/min of N
2O and 20ml/kg/min of O2) is sufficient to keep the patient in a 'Surgical plane of Anaesthesia', at the same time maintaining a normal Pa Co2 level. Result
• Once the patient settles down, Pulse and Blood Pressure remain constant through out the anaesthesia.
• Colour of Blood remains bright red and nail buds, mucous membranes etc. remain bright pink.
• Recovery from anaesthesia is very rapid and uneventful.
